The American Heart Association has an excellent opportunity for a Community Health Lead - CKMH with our National Center.  The location of the selected candidate is flexible and remote.

 

 This is a full-time, benefits eligible, grant-funded opportunity, current funding is through July 2028.

 

The National Community Health Lead for CKMH will be responsible for supporting the local execution of the CKMH initiative’s community strategies. This person will work collaboratively with the CKMH team, local markets, and others on the national Community Impact team to drive impact and ensure the goals of the CKMH initiative are met. The Community Health team for CKMH is responsible for engaging and supporting community organizations in CKMH markets and collaborating with clinical sites to promote clinical-community care coordination and address social needs.

Responsibilities

 

  • Works with the CKMH team to support the development and execution of strategies to achieve goals of the initiative and the organization broadly. Communicates progress to broader community health team and keeps regional and market leadership informed.
  • Cultivates and maintains relationships with community organizations and facilitates collaboration among community organizations and clinical sites.
  • Collaborates with development colleagues to ensure funder deliverables are achieved and all reporting and accountability requirements are met.
  • Collaborates with Association community team members and communicates regularly regarding the CKMH work in markets.
  • Develops resources, leads or facilitates training, and provides technical assistance to support community organizations and volunteers and to support the achievement of CKMH goals.
  • Participates in the National Community Health team and supports activities of the team to advance the mission of the Association.

Qualifications

 

  • Bachelor's Degree in Public Health or equivalent work experience
  • Three (3) years of the following experience:
    • Demonstrated experience in community policy, systems, or environmental change work focused on improving public health with a focus on reducing chronic disease.
    • Demonstrable consulting or advising experience, especially to colleagues over which there is no reporting relationship
    • Demonstrated ability to productively participate in a multi-disciplinary team environment working towards common goals.
    • Advocating for strategies, projects and ideas to internal audiences and helping colleagues develop and achieve outcomes goals
    • Ability to strategically guide and put into practice nationally developed programs in a collaborative business environment
    • Cultivating and managing relationships with community Organizations
  • Sub-grant and grantee management, and budget management

 

Compensation & Benefits

The expected pay range will be $90,000 to $110,000. Pay is commensurate with experience; geographic differentials to the pay range may apply. The American Heart Association reserves the right to pay more or less than the posted range.
