Due to the current pandemic, daily travel is minimal and voluntary to ensure the health and safety of staff. There is no overnight travel currently. In the future, this position may resume daily and/or overnight travel.  All AHA offices are closed and staff are currently working from home.  In the future, this position will resume to being based in the local office.

 

Responsibilities

We have an excellent opportunity for a Community Impact Director based in the San Antonio division of the American Heart Association. The Director will drive the execution of health impact goals within priority markets focusing on the areas of hypertension, cholesterol, diabetes, nutrition, obesity, and physical activity, with a specific focus on diverse and underserved communities.

 

Responsibilities

  • Drive market health assessments, organize and/or lead community collaborations, and build and implement plans for policy, system, or environment change driving toward opportunities for health impact  
  • Build and implement a cross-functional hypertension, diabetes, and cholesterol control strategy with measurable outcomes in the market
  • Collaborate with development staff partners and volunteers to identify, cultivate, and secure program funding, including sponsorship for relevant programs, campaigns, and population health impact opportunities
  • Oversee planning and implementation of sponsored initiatives specific to driving community impact
  • Work with internal staff partners to ensure population health strategies are integrated into development, communications and marketing, advocacy, and healthcare quality improvement activities
  • Recruit, train, and manage volunteers and strategic community alliances to achieve priority community and collective impact goals
  • Work with healthcare providers to ensure best practices for treatment of patients through initiatives and programs including but not limited to: Target: BP™ and Check. Change. Control. CHOLESTEROL™
  • Consult with clinical staff for data entry, data reporting, compliance issues, and strategies for improved patient outcomes
  • Other duties as assigned by manager

Qualifications

Qualifications:

  • Bachelor’s degree from an accredited university
  • 2+ years’ experience working with community/public health issues and/or managing community/public health promotion or related educational programs. Experience in population health a plus
  • Knowledge of the social determinants of health as it relates to behavior modification through self-care activities and policy/system-wide changes
  • Experience working with multicultural and underserved communities preferred. Experience in community led programs, projects, or events
  • Understanding of the benefits associated with use of treatment algorithms and their impact on clinical inertia
  • Proven ability to recruit, mobilize and manage volunteers, including C-suite level executives
  • Ability and willingness to occasionally travel outside the market and to work evenings and weekends as needed
  • Ability to travel within the area and transport items to and from meetings and events
